1. Add your organization
Name, logo, address, contact details, sport and season. Fill it in once and every tool in the toolkit remembers it on this browser.
2. Edit the wording
Click any paragraph and rewrite it. Add sections, delete the ones that do not apply, add signature lines, rename the columns on a scoresheet.
3. Download it
PDF to print, Word if someone else needs to edit it, Excel for anything you will want to sort and total.
Why we give these away
Because the alternative is a locked PDF of someone else's letterhead
Most free sports templates online hand you a generic document with a stranger's formatting and no way to change a word of it. That is not much use to a league director who needs their own name, their own contact details and their own rules on the page.
These tools exist because the person who needs a decent waiver template in August is usually the same person drowning in registration spreadsheets by September, and we would rather be useful first. Every document you build carries a small line at the bottom saying it was made with Playbook Logix. That is the entire cost.
The legal templates in this toolkit are starting points, not legal advice. Waivers, medical releases and background check consent are all governed by state and in some cases federal law. Have an attorney or your insurance carrier review anything you intend to rely on.
Common questions
Are these tools really free?
Yes. Every tool is free to use, with no account, no email address and no limit on how many documents you create. They exist because the people who need a decent waiver template are the same people who eventually need registration software, and this is a more useful introduction than an advert.
Do I have to create an account?
No. Nothing here asks you to sign up or hand over an email address. Your organization details and your edits are saved in your own browser, so they are still there when you come back on the same device.
Where does my logo go?
Nowhere. The logo is read by your browser and used to build the document on your own device - it is never uploaded to us or to anyone else. The same is true of your organization details and everything you type.
Can I change the wording?
That is the whole point. Every template is a starting point: click any paragraph to rewrite it, add or delete sections, add signature lines, and change the column headings on the scoresheets. Nothing is locked.
What file formats can I download?
PDF and Word on every tool, plus Excel on the tools built around a grid - the tryout and player evaluation sheets, the roster, the volunteer sign-up sheet and the equipment checkout form. The PDF contains real, selectable text rather than a screenshot.
Are the legal templates ready to use as they are?
Treat them as a well-built starting point, not as legal advice. Waivers in particular are governed by state law, and several states limit releases signed by a parent on a minor's behalf. Have your attorney or your insurance carrier review anything you intend to rely on.
When the paperwork outgrows the paper
