What is a team roster sheet?

It is the single page a coach keeps in the scorebook: every player on the team, their number and position, and the phone numbers somebody needs if a child is hurt or a parent has not arrived. Leagues, clubs, school teams and tournament staff all keep some version of it, usually printed, usually folded.

It is not the same as a registration record or a medical form. Those live in a system. This one exists to work at a field with no signal, in the hands of whoever happens to be standing there.

One page, in the scorebook, that works without signal

Every coach has a roster somewhere. It is in an app, or a group chat, or a spreadsheet on a laptop at home. What they frequently do not have is the thing they actually need at 6pm at a field with one bar of signal: a single printed page with every player, their number, and a phone number that belongs to an adult.

This makes that page, landscape, on your team's letterhead.

What goes on it

• Player name and jersey number. The number is what officials, scorekeepers and the opposing coach will use.
• Position, if your sport has fixed ones.
• Parent or guardian name: so you are not calling a number and asking for "Jayden's mom".
• A primary phone and a second emergency number. Two, because the reason you are calling is often that the first did not answer.
• A short medical or allergy note. Not the full medical history. The one line that changes what an adult does in the first thirty seconds.

The coaching staff block at the top is worth filling in properly too, including the team parent. When a coach is stuck in traffic, the person who needs to know is usually the one organizing everyone else.

Print two, keep them in different places

One in the scorebook or the first aid bag, one in the assistant coach's bag. The failure mode is not losing the roster. It is the roster being in the car of the one person who is late.

Reprint it when anything changes. A roster sheet with a crossed-out phone number and a new one written in biro is a perfectly good roster sheet; one with a number that quietly stopped working in March is not.

This page is confidential

It carries the names, phone numbers and medical notes of other people's children on a single sheet, which makes it the most sensitive thing most volunteer coaches routinely carry. A few rules worth keeping:

• Give it to coaching staff only, never to the whole parent group.
• Do not post it in a team chat, a shared drive, or anywhere with a link that can be forwarded.
• Do not photograph it on a phone that syncs to a family photo library.
• Never hand a copy to an opposing coach or a tournament desk. If they need a roster, give them names and numbers only. This tool makes that easy: delete the contact columns and download a second version.
• Shred it at the end of the season.

The tournament version

Most tournaments want a roster with names, numbers and dates of birth and nothing else. Rather than handing over your contact sheet, take thirty seconds here: delete the parent, phone and medical columns, add a date of birth column, download, and you have a clean submission roster that gives away nothing it should not.
